Explanation
A meeting of the Disability Review Committee was conducted on April 24, 2002 to review the application for disability retirement for George E. Shaw, Jr. In reviewing the application and supporting information from his physician, it is the recommendation of the Committee for the City Commission to approve George E. Shaw, Jr.'s application.
George E. Shaw, Jr., a Water Plant Operator II who has 4.417 years of service at the GRU Water Plant, has been under the care of physicians for medical reasons. Information provided to the Committee from his physicians (copies of which are available for review in Employee Health Services by the City Commission) indicate that George E. Shaw, Jr. is totally disabled and wholly and continuously unable to perform any and every duty of his employment or of a position to which he may be assigned on the recommendation of the Disability Review Committee.
Fiscal Note
The current monthly salary, estimated or final average earnings of the employees' 36 highest consecutive months of earnings, and the estimated or final benefit for each of these employees has been determined to be:
George E. Shaw, Jr., Current monthly salary: $2,232.65; Estimated average monthly earnings: $2,642.35; Estimated monthly benefit (without Social Security offset, if any determined at a later date): $1,056.94. These benefits shall become effective May 1, 2002.
These benefits will be paid from the Disability Pension Fund.
Recommendation
The City Commission approve the submitted application for disability retirement for George E. Shaw, Jr., Water Plant Operator II, GRU Water Plant.
