Professional Diet/Nutrition Services with Kathryn Parker, Inc. (B)
Explanation
The current contract to provide professional diet/nutrition services expires on September 30, 2004. Since there are no further extensions available, the Risk Management Department recommends that the City Commission designate Kathryn Parker, Inc as a specified source for this service. Kathryn Parker, Inc. has been providing diet/nutrition services for the City of Gainesville's employees and family members since 1991. The last two times the City issued a Request for Proposal, only one response was received. In addition to the past lack of response from competing vendors, the service provided by Kathryn Parker, Inc. have been excellent. Kathryn Parker, Inc. deserves a significant amount of credit for the overall success of the City's wellness program. This contract allows the City to continue a nutrition program as part of the City's Comprehensive Platinum Award winning wellness program, LifeQUEST.
Fiscal Note
The total annual cost of this contract is included in the Risk Management Budget.
Recommendation
The City Commission: 1) designate Kathryn Parker, Inc. as a specified source for diet/nutrition services; 2) authorize the City Manager or designee to negotiate and execute a three-year contract for the period of October 1, 2004 through September 30, 2007; and 3) approve the issuance of a purchase order in an amount not to exceed $45,000 for FY 2005 with the possibility of up to a 3% adjustment factor for each of the two subsequent fiscal years.
